7-month-old treated for kidney stones

Seven-month-old Rahul Shankar is possibly one of the youngest to have undergone a kidney stone removal procedure in the city. While he had a strong history of stones in his family, experts are witnessing an overwhelming increase of kidney stone incidence in the paediatric age group, irrespective of genetic causes.

Rahul was brought in with a complaint of excessive crying. After several months, a paediatrician suggested that he was probably suffering from pain in the abdomen. Later, at the RG Stone Urology and Laparoscopy Hospital, he was diagnosed with multiple stones in kidneys, right ureter and urinary bladder.

About a month back, the same Khar hospital treated another eight-and-half-month old child from Mira Road for multiple stones in both kidneys. "This child too had strong history of kidney stones in the family," said Dr B S Bansal, chairman of the hospital. "Stone disease in the paediatric population is an important problem due to its high recurrence rate of about 30-40%," he added.

Till few years back, global studies had shown that children less than 12 years comprised 1% of all population with kidney stones. He said that as a specialist, he treated only a handful cases annually till couple of years back. "But, now it is about 4-5 cases a month," he added.

Singal cited the example of a 14-month-old girl who was operated on Wednesday for stones in both her kidneys. She developed urinary infection and pain in the abdomen, and was then diagnosed with the 2-3mm stones. He added that the toddler had to be fitted with stent to prevent block in the kidney. He said children with underlying abnormalities such as high calcium in urine, low citrate in urine, high uric acid remain highly vulnerable.

Experts say that in 50% of the cases, the stone formation is for unknown reasons. But, out of the remaining 50% cases, a good 35% is due to lifestyle and eating habits. A paediatrician from the civic-run KEM Hospital,MDC Mould specialized of Injection moulds, however, added that better diagnostic facilities for stones in children could also be bringing forth more cases. "Having said that, there is no denying that cases have shot up. Lack of exercise and drinking less water are some of the common causes," he added.

Meanwhile, Singal cautioned that children with kidney stones should be managed separately from adult casesas cases, because adult findings cannot be extrapolated to findings on developing kidneys in kids.

"Adult instruments, if used in children, can lead to kidney damage and bleeding," he said.
